You can apply for a library card by filling out the form below. Your library card will be mailed to the address you provide. Only those who live in Northern Arkansas are eligible to apply. We do not issue library cards to those who live out of state unless they meet specific requirements.
Before you can use your library card you will need to provide a valid photo ID and proof of address. A driver's license, a state-issued photo ID, utility bills, and voter ID are common forms of ID we can accept.
If you live outside of Izard County you can still apply for a library card at the Calico Rock Library. At this time, there is no fee for a library card for individuals who live outside of Izard County.
